What Concerns Can the Lower Eyelid Recontour Program Address?

Under-Eye Bags Under-Eye Bags

Addresses protruding lower-eyelid fat or under-eye bags that create a tired appearance.

Deep Tear Troughs Deep Tear Troughs

Assesses deep under-eye hollows and the orbital rim to plan a smoother contour.

Loose Skin and Wrinkles Loose Skin and Wrinkles

Appropriately addresses excess skin and laxity of the lower eyelid.

Under-Eye and Mid-Cheek Transition Under-Eye and Mid-Cheek Transition

Creates a soft transition from the under-eye to the cheek without making the eyes look hollow or harsh.

Balanced lower-eyelid structure

Balancing the Lower-Eyelid Structure

Many people think lower-eyelid surgery simply removes under-eye bags. In reality, concerns often involve several factors together, including protruding fat, loose skin, deep tear troughs, the orbital rim, and mid-cheek volume loss.

“The goal is not to make the under-eye area as flat as possible, but to create a brighter, smoother contour that preserves the face’s natural beauty.”

Why Do Under-Eye Bags, Hollows, and Wrinkles Develop?
GeneticsNatural facial anatomy may create prominent under-eye bags or hollows.
AgingThinner skin and a weakened fat-supporting membrane make under-eye bags more visible.
Allergies or Frequent Eye RubbingCan make the under-eye area dark, swollen, and tired-looking.
Under-eye bags and wrinkles
Lifestyle FactorsLack of sleep, stress, dehydration, or eye strain can make the under-eye area look tired.
Certain Medical ConditionsSome conditions can cause under-eye swelling and should be assessed by a doctor.

Why Skincare May Not Be Enough in Some Cases

Skincare, rest, and selected treatments may improve hydration or some darkness. However, when the main concern is protruding fat, loose skin, or a structurally deep tear trough, external skincare alone may not adequately address it.

What Is the Lower Eyelid Recontour Program?

The Lower Eyelid Recontour Program is lower-eyelid surgery designed to address under-eye bags, tear troughs, laxity, and wrinkles. It evaluates not only localized fat but also the relationship between the under-eye area, mid-cheek, and overall facial structure.

What Does the Surgeon Assess Before Surgery?

What appears to be an under-eye bag may involve protruding fat, deep hollows, loose skin, mid-cheek volume loss, or lower-eyelid laxity. Planning therefore considers multiple components rather than only the visible surface concern.

Amount of protruding lower-eyelid fat
Skin quality and under-eye laxity
Lower-eyelid support and strength
Tear-trough shape and orbital-rim anatomy
Relationship between the under-eye and cheekbone
History of allergies, dry eyes, or frequent eye rubbing
History of under-eye filler injections
Brow and upper-eyelid position, including temple laxity
Lower-Eyelid Recontouring Techniques
Transconjunctival incision technique

Transconjunctival Incision

Suitable for patients with prominent lower-eyelid fat but minimal skin laxity, often younger or middle-aged patients whose main concern is protruding fat.

External incision beneath the lower lash line

External Subciliary Incision

Suitable for patients with both under-eye bags and loose skin, such as prominent wrinkles, excess lower-eyelid skin, or lower-eyelid laxity.

Lower-Eyelid Recontouring Procedure

1

Personalized Assessment and Planning

Assess under-eye fat, excess skin, lower-eyelid laxity, and under-eye hollows.

2

Prepare for Surgery

Clean the eye area, administer local anesthesia, and use light sedation in selected cases when appropriate.

3

Create the Incision Using the Selected Technique

The incision may be inside the eyelid or just beneath the lower lash line, depending on the patient’s concerns.

4

Address Excess Fat, Skin, and Muscle

This may involve removing selected fat, repositioning fat, or smoothing loose skin.

5

Close the Incision Meticulously

External incisions are closed carefully, while internal incisions generally leave no externally visible scar.

For the first 1–2 nights, sleep with your head elevated to reduce swelling.
Keep the incision dry during the first 3 days.
For the first 1–2 weeks, avoid eye makeup, contact lenses, and strenuous exercise.

Frequently Asked Questions (FAQ)

This program does more than remove fat. It evaluates the under-eye area holistically, including fat pads, tear troughs, loose skin, the muscles around the eyes, and the transition to the mid-cheek, so the technique can be tailored to each patient.
Not always. Lack of sleep can temporarily make the under-eye area look puffy or dark, but persistent under-eye bags are often related to lower-eyelid fat, skin laxity, or facial anatomy and cannot be corrected by rest alone.
It depends on the selected technique. A transconjunctival incision is placed inside the eyelid and leaves no externally visible scar. An external incision sits just beneath the lower lash line and generally becomes discreet after healing.
Swelling and bruising are usually most noticeable during the first 3–7 days and gradually improve over 2–3 weeks. Recovery varies with the technique used and each patient’s postoperative care.
